The Sudbury Hospice provides an integrated model of palliative care to its residents through a team-based approach to care delivery that includes physicians, nurses, pharmacists, social workers, physiotherapists, hospice volunteer visitors, spiritual advisors and bereavement counsellors.  It brings family members, friends and healthcare professionals together as a care giving team so residents can live their remaining days in dignity and comfort, surrounded by the people who love them.

 

• Provides 24 / 7 nursing and personal support care to end-of-life residents.

         - Resident Care Coordinator, RNs, RPNs & PSWs 

• MOHLTC provides $627,300 annually for nursing and personal support services.

• Homelike environment with 10 private bedrooms, with ample space for a dining/living room, children/family room, sacred space, offices, storage and laundry facilities.
