ORIGINAL MANDATE
COMMUNITY RESIDENTIAL HOSPICE DEFINITION
A community residential hospice is a home-like environment where adults and children with terminal illnesses receive end-of-life care services. It provides individuals, who wish to die outside a hospital environment and who cannot be cared for at home, with compassionate care and comfort in the last stages of their lives
VISION
To provide consistent, comprehensive and high-quality end-of-life care to palliative individuals and their families/caregivers in a bilingual residential hospice setting.
PURPOSE
The Sudbury Hospice Steering Committee will represent all hospice palliative care stakeholders in the Manitoulin-Sudbury District. It will facilitate the planning, implementation and development of a bilingual community residential hospice in Sudbury to ensure the delivery of a coordinated, comprehensive and accessible system of end-of-life care in a residential hospice environment.

- Review and Revise Draft Terms of Reference
- Develop Mission & Vision
- Develop Policies & Procedures
- Develop Partnership Agreements
- Develop Capital Funding Requests
- Develop Ongoing Operational Costs Fundraising Campaign
- Develop Property & Building Design
- Develop a Communication & Marketing Plan
- Develop a Governance Model for the Future CRH
- Develop Programs & Services
- Review Budget and Operating Costs
- Advocacy
- Other
ACCOUNTABILITY
The SHSC is accountable to the Board of Maison La Paix and to palliative residents, their caregivers and the community.
MEETINGS
The Steering Committee will meet on a monthly basis.
WORK GROUPS
The Steering Committee will strike work groups as required. Members-at-large and representatives from community partners will be invited to join these groups. Work groups will make recommendations to the Steering Committee and are accountable to this Committee.
